Notification of Practice Changes
Dr. Lasser has some exciting news to share with you!
On Monday, Nov. 4, 2013, I will be joining Johns Hopkins Community Physicians (JHCP) Howard County practice where I will be able to provide expanded services for your family. Johns Hopkins Medicine is a national leader in health care and I am excited to be joining this respected institution to continue to provide your family with enhanced quality patient care. I will begin seeing patients at JHCP Howard County, located a quarter mile away from my current location at 6350 Stevens Forest Road in Columbia.
I have been very fortunate and privileged to have been able to serve your family for many years and I am excited to continue this for many more years. I am very grateful to all of my patients who have entrusted me with the care of their children and I intend to continue providing the highest quality care to all of my patients at this new location.
JHCP Howard County’s hours are 8:30 a.m. to 5 p.m. on Monday, Wednesday, Thursday and Friday, and 11:30 a.m. to 8 p.m. on Tuesdays. This practice also offers other specialists including family practitioners, internists and obstetricians-gynecologists so that the entire family can receive care in one location, should they so desire. To schedule an appointment for November and beyond, please call 443-259-3770.
In order to provide seamless care, please complete the attached medical release authorization (see the "RECORD RELEASE" web page) and return it to my current office before Sept. 25 or to JHCP Howard County after Nov. 4. This is necessary because, even though I will still be providing your medical care, this change is considered a transfer to a different practice organization. Additionally, please inform your pharmacist that your doctor has moved when requesting refills from the pharmacy.
JHCP Howard County accepts many insurance plans, including most of those which my practice currently accepts. . Please check with the JHCP practice office or the JHCP website to ensure that your insurance plan is accepted. To view a list of participating insurance plans online, go to www.hopkinsmedicine.org/jhcp. Under “For Patients” in the left menu bar, click “Insurance Plans.” Medical Assistance patients should call their case manager and request a change to Priority Partners as this is the only MCO that Johns Hopkins accepts. Call the JHCP office if you have any questions.
If you are unable or do not wish to continue care with me, I strongly encourage you to obtain your medical records from my office before Sept. 25. Wherever you choose to receive your child’s care, all patients must complete the medical records release found on the RECORD RELEASE link to the upper left. Records may be mailed or picked up; however, they will not be faxed or emailed to ensure confidentiality of patient information.
